[Choose #1] Fin strap too large, or foot too small, lost a fin a bunch of times. Changed straps and fixed the problem .

[Choose #1] In Belize, my first stage reg had a steady stream of bubbles coming from it. Bad O ring,and was at 110 feet on a whale shark dive.

[Choose #1] Bcd 1size too big dry suit that was an extra large I should have been a large. Wireless dive computer froze up after 1 dive

[Choose #1] Of course, gotta dive... regulators that don’t shut off completely, and BCs with leaky valves, BCs that require manual inflation, cmputers that go out (always try to have manual gauges along with my computer also). Always fixed asap.

[Choose #1] I was certified in 1967 and have over 3000 logged dives. It was never intentional but sometime shit happens. I try not to let it happen often.

[Choose #1] Octo began to leak, so I switched to it to make use of the escaping air. Replaced octo for subsequent dives.

[Choose #1] Malfunctioning BC inflator, inflated orally, wasn’t a big deal.

[Choose #1] Never intentionally(w/ my own gear), but with rental bc, on drift dive, inflator hose came off, hard to stay positive in the waves we had. Thank goodness for buddy, made it a lot easier

[Choose #1] Diving the spar off of morehead city, nc. my inflator hose crapped out. i had to orally inflate when needed. no big deal turned out to be a great dive.

[Choose #2] Never started the dive, knowing something is wrong with equipment (i believe that was a question). One incident I had was flooded comp at the end of the dive (shallow dive, cont. only for next 10-12 min including safety stop). Diving w/ 2 comp since.

[Choose #1] I was at Dreams La Romana in the Dominican Republic on my second OW checkout dive when the first stage cracked(really not sure how the hell that happened) and I was out of air in no time. Luckily my dive instructor was only a few kicks away.

[Choose #1] Very minor. In the Bahamas, we got down to a 100 fsw and I twisted my SPG to check and a seal blew (rental gear.) It wasn’t all that bad, so we just kept going and I kept a close eye on it. Plenty air at the end of the dive.

[Choose #1] Just 3 weeks ago I was in a Cavern dive and both of my regs started leaking and my tank all around a 2 minute period. That is some fear thrown on you in all of 2 seconds.

[Choose #1] Twice, once with an inflator hose that broke and caused me to be air free for the dive and the other with a reg failure. That one was interesting...

[Choose #1] O-ring on inflator hose blew and was leaking air when hose attached, disconnected hose and made dive with, orally re-inflated orally at end of dive on the surface as weighting was perfect and no air needed throughout the dive.

[Choose #1] Beach dive once with a broken inflator, keep in mind when I started diving we didn’t have power inflators so not a big deal for me.

[Choose #1] But didn’t knowingly start the dive with defective gear. Once with a BC that wouldn’t stop inflating at depth, but just removed the hose.

[Choose #2] I once rented a BCD in Belize, it was ripped and worn, after that I went home and bought one!

[Choose #1] Almost killed me. During my AOW night dive a grain of sand in my "RENTAL" BCD inflator button stuck "on" and I shot to the surface from 30 feet deep in about 5 seconds. I yelled out "SHIIIIIITTTT!" the whole way to avoid exploding my lungs.

[Choose #1] Yep, first stage reg failed when I was doing my deep dive speciality... Luckily buddy was very close. Regs sent off for testing and T pin was not installed correctly and buckled the first time they were used, causing the first stage to fail

[Choose #1] I have dove with a rented BC that was slowly leaking air, I just added a bit of air into it when needed.
